10/15/2023 EnchantaVaughn Approved Behavior: QMPHA greeted Ms. Brown upon arrival to the home setting. Upon arrival to the home setting, Ms. Brown appeared to be in a good mood evident by her facial expressions such as a smile on the face. QMPHA inquired about her day. Ms. Brown discussed with QMPHA that she was in a good mood today. No visible signs of SI/HI during the session. Intervention: QMPHA began the session by assessing Ms. Brown’s emotional well-being. QMPHA reinforced the importance of Ms. Brown paying her bills in a timely manner. QMPHA reinforced the importance of being accountable/responsible for her choices/decisions. QMPHA continued to educate her on the benefits of taking her psychotropic medication as prescribed (stabilizing her moods and overall mental health). QMPHA encouraged Ms. Brown to identify how her decision to remain in relationships/friendships with individuals that have negatively impacted her life. QMPHA reinforced the importance of building/maintaining a healthy relationship with others and encouraged Ms. Brown to identify her inability to disassociate herself with individuals that negatively impact her life. Response: Ms. Brown appeared to be receptive to QMPHAs’ suggestions that were provided to her by QMPHA. Ms. Brown informed QMPHA that she has made the decision to resume taking her psychotropic medication as prescribed. She shared that she now understands that it really does stabilize her moods and mental health. Ms. Brown admits that she makes poor choices/decisions daily; however, she is about to start thinking things through as well as considering the consequences of her actions.
